Ann Raikes Hartley is a board-certified Advanced Practice Registered Nurse with over 25 years of nursing experience specializing in Primary Care and experience as an Adult Nurse Practitioner for the last 8 years. She graduated from the University of Central Florida with her Doctoral degree in Nursing. Ann is a member of Central Florida Advanced Nurse Practice Council (CFANPC), Florida Nurse Practice Council (FNPN), and the American Association of Nurse Practitioners (AANP). She has also earned the following certifications: ANP-BC, CNL (Clinical Nurse Leader). In her role as an APRN, Ann firmly believes that CenterWells senior-centered healthcare model truly benefits her patients because it allows the Care Team more time to focus on the complex healthcare needs of the elderly population. Ann believes that because CenterWell is a medical home, it ensures that information from the specialists is available to the entire Care Team, which helps her further explain to her patients the entirety of their care. What she enjoys most about being an APRN, is the ability to make a difference in the lives of her patients every day, from helping reduce health disparities to increasing health literacy. Ann always emphasizes how CenterWell embodies the ultimate goal of healthcare, because it allows the Care Team to care for the patient as a whole as they support them along their health journey. Ann defines the right relationship with her patients, as one that is based on mutual trust and respect. She maintains this type of relationship with her patients by taking the time to listen to their needs and concerns, while providing them with professional, compassionate care. During her spare time, Ann enjoys reading, shopping, as well as attending festivals and watching singing/dancing reality shows.
