Isuan Iriabho, APRN

Ms. Isuan Iriabho, MSN, CRNP, AGPCNP-C, GS-C, is an experienced Adult-Gerontology Primary Care Nurse Practitioner with a strong focus on value-based care and chronic disease management. She earned her Master of Science in Nursing from the University of Alabama at Birmingham and a post-graduate certificate in nursing education from the University of Alabama at Huntsville where she is also currently pursuing a Doctor of Nursing Practice at the University of Alabama at Huntsville. Ms. Iriabho has provided care across diverse settings, including in-home primary care with Evernorth Health Services, hospice care with Gentiva, and long-term care facilities through Optum. Her clinical expertise emphasis on improving patient outcomes and reducing hospital readmissions. She has also served as an adjunct clinical instructor at Bevill State Community College, mentoring nursing students in real-world clinical environments. Outside of her clinical work, Ms. Iriabho enjoys doing volunteer work, listening to music, and spending time with family.
