Access to transportation is vital to one’s health and well-being, as well as a major factor in maintaining independence and social connections—especially for older adults. If lack of transportation has prevented you from going to the doctor, pharmacy or grocery store, or kept you from connecting with family and friends, CenterWell can help connect you with transportation resources to get you to where you need to be.

When it comes to important outings like getting to and from medical appointments, accessing healthy foods and staying connected to loved ones, we put our patients’ needs at the center of everything we do. CenterWell can help you find transportation resources in your community. You may also consider asking the following:

 

  • If your health insurance or Medicare plan helps cover transportation
  • If a family member, friend or a caregiver can assist with transportation
  • If your place of worship offers transportation services for seniors

Designed with seniors in mind, we work together to assist with all of your health and wellness needs. There are also other resources and support which may be available to you, including: 

 

  • Your medical insurance or Medicare plan - which may cover non-emergency medical and non-medical transportation—check your individual plan for benefits
  • Eldercare and area agencies on aging - addresses the needs and concerns of older adults at the regional and local levels, including transportation challenges: 1-800-677-1116 (TTY 711), www.eldercare.acl.gov
  • National Transit Hotline - connects you to local transit providers who receive federal funds to provide transportation to seniors and individuals with disabilities: 1-800-527-8279 (TTY 711), www.transit.dot.gov
  • 211 Helpline Center - provides community information and referrals to social services in times of crisis and for everyday needs, including transportation: www.211.org or dial 211 from any phone

 

We’re here to help guide patients in securing a safe and reliable means of getting to and from their various providers and other important destinations. If your health insurance or Medicare plan offers a transportation benefit, we can coordinate rides to and from appointments.
