Wendy Swayne, APRN

Ms. Wendy Elizabeth Swayne, MSN, AGNP-BC, is an Adult/Geriatric Nurse Practitioner dedicated to providing attentive, patient-centered care. She earned her Master's in Nursing from the University of Illinois at Chicago. Her experience included delivering primary care for adults and seniors through in home visits, telemedicine, and community-based services. She provided chronic disease management, preventive screenings, Medicare wellness visits, and care coordination across multidisciplinary teams. Earlier roles spanned mobile community health, primary care, and extensive nursing experience in gastroenterology, emergency intake, and medical-surgical units. Ms. Swayne also served as a preceptor and charge nurse, mentoring clinical staff and supporting team development. She believes in treating patients with the same dedication she extends to her own family and is passionate about helping individuals advocate for their health. Outside of work, she enjoys spending time outdoors, traveling, and being with family.
