Thomas Edward Sulkowski, MD

Dr. Thomas Edward Sulkowski is a board-certified family medicine physician with experience delivering comprehensive care across all age groups, with a strong emphasis on geriatric and chronic disease management. He earned his Doctor of Medicine from the Medical College of Virginia at Virginia Commonwealth University and completed his residency at Chesterfield Family Practice Residency Program. Dr. Sulkowski is the founder and owner of Sulkowski Family Medicine in Murfreesboro, Tennessee, where he provides a full spectrum of care including preventive services, chronic disease protocols, and minor procedures. His practice is recognized as a Level 3 Medical Home. He has also served in leadership roles, including Interim Medical Director at Primary Care Medical Home for Calsonic Corporation. Earlier in his career, he provided care in various clinical and emergency settings across Tennessee, Georgia, and Virginia. Outside of medicine, Dr. Sulkowski enjoys enjoying the outdoors, spending time with family, and traveling.
