Susan Lephart, APRN

Ms. Susan Lee Lephart, MSN, APRN, FNP-C, is a Family Nurse Practitioner dedicated to adult and geriatric care. She earned her Master of Science in Nursing and built a career centered on supporting patients through the aging process with compassion and individualized attention. Throughout her professional experience, Ms. Lephart provided primary and preventive care, managed chronic conditions, and emphasized lifestyle-focused strategies to support long-term wellness. She brought particular expertise in diabetes management, nutrition, and exercise, and valued the opportunity to learn about each patient's unique needs to help them live their best life through their senior years. Her philosophy of care focused on treating patients with the same time, dedication, and commitment she would offer those closest to her, while fostering trust and active patient engagement. Outside of work, she enjoyed spending time with family, volunteering, and being outdoors.
