Riana K Reid, APRN

Ms. Riana Kiah Reid, MSN, APRN, FNP BC, CCM, is a Family Nurse Practitioner with more than 12 years of nursing experience spanning hospice, home health, oncology transitions of care, and primary care case management. She earned her Master of Science in Nursing with a Family Nurse Practitioner focus from Purdue Global University after completing her Bachelor of Science in Nursing at Chamberlain University. Ms. Reid has cared for seniors across home health, hospice, and coordinated-care settings, supporting dignity, comfort, and independence for patients with complex medical and social needs. She has managed symptom control, guided families through end of life decisions, coordinated safe transitions of care, and strengthened continuity across interdisciplinary teams. Guided by a philosophy that emphasizes listening and building trust, she is passionate about diabetes management, heart health, and addressing food insecurity. Outside of work, she enjoys spending time with family, listening to music, and reading.
