Rachel Ann Freitas, APRN

Ms. Rachel Ann Freitas, MSN, APRN, FNP-BC, is a Family Nurse Practitioner with experience supporting adult and senior patients through hospice, primary care, and women's health. She earned her Master of Science in Nursing from Anderson University after completing her BSN at Lander University. Her work included providing symptom management, end of life support, patient education, and care coordination across interdisciplinary teams. She also delivered obstetric care in high acuity hospital settings and supported chronic disease management through compassionate, evidence based care. Ms. Freitas held leadership roles during her nursing education and emphasized listening closely to patients to enhance their quality of life. She is passionate about exercise, heart health, and nutrition, and has a warm commitment to preserving the well being of seniors. Outside of work, she enjoys spending time with her family, exploring the outdoors, and visiting Dollywood.
