Melissa M Baltazar, APRN

Ms. Melissa M. Baltazar, DNP, FNP-BC, AGNP-BC, is a family and adult-gerontology nurse practitioner dedicated to preventive care and whole-person wellness. She earned her Doctor of Nursing Practice from the University of San Diego. Ms. Baltazar brings extensive clinical experience from her years as a registered nurse in a progressive care telemetry unit, where she also served as a charge nurse and developed strong leadership skills while caring for medically complex patients. Her advanced practice experience includes clinical training across family medicine, gerontology, cardiology, internal medicine, and palliative care, as well as providing cardiology care in both interventional and electrophysiology settings. She is especially passionate about supporting underserved and aging populations, helping patients maintain independence and optimize quality of life. Guided by a philosophy rooted in compassion, education, and patient advocacy, she works to ensure every patient feels heard and empowered in their healthcare journey. Outside of work, Ms. Baltazar enjoys traveling, staying active with fitness classes, and sailing with her family and dogs.
