Manuel Lazaro Falcon Valdes, APRN

Manuel Lazaro Falcon Valdes, FNP, has joined CenterWell as a board-certified family nurse practitioner. Passionate about heart health, exercise, nutrition, and loneliness/socialization, Mr. Falcon Valdes earned his Master of Science in Nursing from South University in Savannah, GA, and his Bachelor of Science in Nursing from Sacred Heart University in Puerto Rico. Earlier, he completed his medical degree and specialization in clinical ophthalmology at the Higher Institute of Medical Sciences of Matanzas, Cuba, and was recognized with the prestigious Gold Title Award. He worked for two years as a primary care physician in Venezuela and then relocated to the U.S. Before joining CenterWell in 2023, Mr. Falcon Valdes worked as a family nurse practitioner at Navigation Medical Center and Panamericana Family Medicine Clinic and as an RN at Gramercy Outpatient Surgery, New Horizon Hospital, and Consultorio Medico Jones, all in Houston, TX. His affinity for working with seniors stems from his experience providing home health services, where he witnessed their unique needs. He believes listening to his patients is critical to improving their health and quality of life, a care philosophy aligned seamlessly with CenterWell's patient-centered values. Mr. Falcon Valdes chose healthcare out of a passion for helping others and serving his community. Outside of work, he enjoys spending time with his family, performing music, and volunteering.
