Juvelyne De Guzman, APRN

Mrs. Juvelyne Yap De Guzman, MSN, APRN, AGNP-C, CFNMP, specializes in adult-gerontology primary care. She earned a Master of Science in Adult Gerontology from Maryville University. Mrs. De Guzman brought more than 20 years of healthcare experience, caring for adult and geriatric patients in primary care, home-based, long-term, and post-acute settings. She managed chronic conditions, preventive care, wellness visits, and transitions of care while helping patients maintain independence and quality of life. She also served in leadership and mentoring roles, supporting clinical teams and new providers. Mrs. De Guzman believes in compassionate, patient-centered care built on listening, education, and shared decision-making. She is passionate about nutrition, brain health, exercise, diabetes prevention, and healthy aging. Outside of work, she enjoys traveling and spending time with her family.
