Jason Buchanan, MD

Dr. Jason N. Buchanan, MD, MS, FAAFP, FACHE, is a triple board-certified Family Medicine Physician with over 15 years of clinical and leadership experience. He earned his medical degree from Baylor College of Medicine and holds a Master of Science in Biomedical Informatics from UTHealth, with ongoing MBA studies at Johns Hopkins University. Dr. Buchanan has served as a practicing clinician and Assistant Professor at Baylor College of Medicine within Harris Health System, one of the largest safety-net systems in the U.S. He has provided full-scope outpatient care to diverse populations, including older adults, and has been recognized for excellence in patient satisfaction and clinical outcomes. His expertise spans clinical informatics, digital health, and healthcare management, with a focus on improving care delivery through technology and data-driven strategies. Dr. Buchanan is also a published author and frequent speaker on health IT and population health. Outside of medicine, he enjoys the outdoors and spending time with family.
