Holly J Lago, APRN

Ms. Holly Jo Lago, MSN, APRN, FNP-C, specialized in family practice and primary care. She earned her Master of Science in Nursing from Minnesota State University in Mankato, Minnesota. With nearly two decades of experience in hospital, clinical, surgical, and administrative settings, she provided care in family practice, orthopedic medicine, and chronic disease management. Her roles included serving as a Family Nurse Practitioner at Oak Street Health and Prime Healthcare, as well as leading telemedicine initiatives for chronic care management. Holly also held leadership responsibilities, mentoring providers and managing ambulatory surgery center operations to ensure quality and compliance. Guided by her philosophy of embracing a culture of caring for people regardless of age or circumstances, she is passionate about heart health, diabetes, nutrition, and promoting social connection. Outside of work, she enjoys cooking, reading, listening to music, and spending time outdoors with her family.
