Hannah Elizabeth Moorhead (Sloger), APRN

Ms. Hannah Sloger Moorhead, DNP, APRN, FNP-C, is a dedicated Family Nurse Practitioner with a strong background in nursing and patient care. She earned her Doctor of Nursing Practice from the Medical University of South Carolina and her Master of Science in Nursing from Clemson University. Hannah is certified as a Family Nurse Practitioner and has extensive experience in primary care settings. She currently provides comprehensive care at Palmetto Primary Care Physicians, treating patients across the lifespan and managing both chronic and acute conditions. Her previous roles include serving as a Family Nurse Practitioner at CVS Minute Clinic and as a Registered Nurse in the Medical ICU at Atrium Health Carolinas Medical Center, where she gained valuable experience in acute care and COVID-19 patient management. Hannah is also involved in research, having conducted studies on hospital-acquired pressure injuries and the effects of mammography radiation on breast cancer severity. Outside of work, she enjoys the outdoors, reading books, and spending time with family.
