Derrien Darshay Baker, APRN

Ms. Derrien Darshay Baker, MSN, APRN, FNP-C, is a Family Nurse Practitioner with experience spanning primary care, community health, inhome assessments, women's health, and chronic disease management. She earned her Master of Science in Nursing from Nicholls State University. Ms. Baker has provided care to adults and seniors in underserved and high volume settings, managing acute and chronic conditions, conducting preventive screenings, and performing procedures such as incision and drainage, joint aspiration, and wound care. She has supported older adults across clinical and community based environments, helping them navigate complex medical and social needs. Guided by a philosophy rooted in listening first, she focuses on realistic, evidence based care plans that patients can follow, emphasizing prevention, education, and shared decision making. She is passionate about heart health, brain health, and promoting exercise as part of long term wellness. Outside of work, she enjoys traveling, cooking, and home improvement projects.
