Chelsey Sundberg, APRN

Ms. Chelsey Sundberg, CNP, is a certified nurse practitioner specializing in adult geriatrics and internal medicine. A native of Dell Rapids, South Dakota, she completed her nursing education at Presentation College and the University of Cincinnati. Ms. Sundberg has focused her career on caring for adult and older populations, supporting prevention, chronic disease management, and overall wellness. Her approach to care emphasizes empowering patients to take an active role in their health by integrating holistic practices with evidence-based medicine to promote healthier lifestyles and long-term well-being. She is dedicated to building strong, trusting relationships and helping patients achieve optimal health through education and thoughtful guidance. Outside of work, she enjoys spending time with her family as a wife and mother of three young children.
