Frequently asked questions

CenterWell™ Senior Primary Care offers senior-focused primary care with compassionate physicians supported by a dedicated Care Team. This allows us to provide our patients proactive care, no-rush appointments, same day appointments and timely follow-ups. Read more about our services and how we’re different.

To become a patient, you can complete this form on our website and a team member will reach out to help and answer any questions you may have. You can also call or visit a CenterWell Senior Primary Care location near you, where you will receive a new patient information packet that you can bring to your first appointment.

 

If your medical records are with another physician, we will ask you to complete a Medical Records Release form that gives your new CenterWell Senior Primary Care doctor permission to request the records.

Arrive early to your first appointment to fill out your new patient paperwork or download the forms here and complete them at home.

In addition to your forms, you should bring:

 

• Your Photo ID (such as a driver’s license or a passport)

• Your insurance card

• A list of your current medications

Please call 866-405-2821 to schedule an appointment. Same-day appointments are usually available in case of an urgent need. To schedule lab appointments, please call us in advance to avoid longer wait times.

Our doctor-led Care Team is available by phone 24 hours a day, seven days a week. Patients can call 866-405-2821 to talk to a health professional after hours. CenterWell Senior Primary Care patients have the option to see their doctor remotely with a TeleVisit. TeleVisits are virtual video visits. They are private, secure, and allow you to meet with your provider one-on-one from home or wherever you’re most comfortable. TeleVisits can be accessed from the patient portal or via a link that is texted or emailed to you.

We accept a variety of insurance plans that vary by location. To learn more about the plans we accept, including how to switch to an accepted plan, please visit our location finder. You can also find contact information in case you need more help.

To change doctors, you can call 866-405-2821 and one of our care team members can guide you through the process. The most important things to consider are whether the potential new doctor is accepting new patients, whether they accept your insurance, and whether you feel comfortable with them.
