Ifeoma Ukachukwu, Nurse Practitioner

Ifeoma Ukachukwu is a board-certified family and psychiatric mental health Nurse Practitioner focused on providing comprehensive care to the senior patients at CenterWell. She completed her Master of Science in Nursing at the Downstate Medical Center in Brooklyn, NY and later her doctorate in Nursing from the Rocky Mountain University in Utah. She began her nursing career 13 years ago in the hospital while focusing on geriatrics, adults, mental health, ICU and emergency nursing and later served as a Clinical Advisor and Nursing Supervisor before making her way to CenterWell. As a family nurse practitioner, Ifeoma has been trained to treat the whole family from infancy to the elderly. That means she can assess, diagnose and treat the majority of conditions that are presented at CenterWell and can refer patients with complex conditions to a physician or a specialist. Having also worked in numerous nursing homes and outpatient care clinics, Ifeoma can provide holistic care to her patients and enhance their living standards with a comprehensive yet customized healthcare service. “My passion is in geriatrics…I am also focused on teaching patients about maintaining a balance for optimal health.
