Meet your Care Team

We’re more than providers – we’re advocates for you and the way you want to live. Outside of your provider, we have a whole team supporting you:

 

  • Care coaches: nurses who work with your provider to develop your care plan. 
  • Clinical pharmacist: health professionals who specialize in administering and providing advice about medication.
  • Resource Coordinator: social worker who works to find services related to transportation issues, housing, finance, food and in home services as needed.
  • Behavioral health specialist: clinical social worker who works with patients to recommend treatment for behavioral or mental health needs.
